Executive Summary

The Multi-Assistance Center (MAC) demonstration project grant will fund a service model thatprovides single point of contact coordination, medical, therapy, and non-medical services for individuals of all ages with special needs, including Intellectual and Developmental Disabilities (IDD). Single Point of Contact Coordinators, or SPOCCs, will be both the person in direct contact with the individuals and their families and the primary coordinator of services for both in-house services and community partners. SPOCCs are critical to the MAC service model and add value through their broad knowledge-base of services in their region, which they use to help individuals and their families gain information and navigate access to services.
